Understanding Incisions and How They Determine Your Scar
The location of your boob job scar is determined by the incision approach we choose together during your consultation. This isn’t a one-size-fits-all decision—it’s a carefully considered choice based on your unique anatomy, the type and size of implant you’re selecting, your aesthetic goals, and even your lifestyle preferences.

My goal is always to place your incision where it will be least noticeable—typically in natural creases or transitional areas where your eye doesn’t naturally focus. While the incision lines are permanent, they fade significantly over time and, with proper placement and care, often become barely perceptible. Let me walk you through each option so you understand what to expect.
Inframammary Incision (Breast Crease)
This is the approach I use most frequently, and for good reason. The inframammary incision is made in the natural fold where your breast meets your chest wall—that crease you can feel when you lift your breast. It is often considered the gold standard in modern breast augmentation.
The scar sits tucked within this fold, which means it’s naturally hidden by the overhang of your breast tissue itself, especially when you’re standing or wearing a bra. When you’re lying down, the scar may be visible, but it’s typically a thin line that fades to match your skin tone over time.
What makes this approach so versatile is the excellent, direct access it gives me to the surgical site. This clear view allows me to create a precise pocket for the implant, control any bleeding carefully, and position your implant with unparalleled accuracy. It works beautifully with both saline and silicone implants of any size. Furthermore, if you ever need a revision surgery down the road—to change implant size or address a complication—I can almost always reuse the same incision site, preventing the creation of new scars.
Because this incision doesn’t go through breast tissue or near the nipple’s nerve supply, it generally preserves nipple sensation and has the lowest risk of impacting your ability to breastfeed in the future. The scar itself is typically one to two inches long—just enough to insert the implant safely and without undue stress on the implant shell.
Periareolar Incision (Around the Nipple)
The periareolar incision follows the lower semi-circle of your areola, right where the darker pigmented skin meets the lighter breast skin. This natural color transition is what makes this scar placement so clever—it acts as a form of camouflage, allowing the scar to blend in beautifully as it heals.
This approach gives me good access for precise implant placement, and many patients love that the scar essentially disappears into the natural boundary of the areola. However, it’s not the right choice for everyone, and it carries specific considerations.
If your areolas are quite small (typically less than 3-4 cm in diameter), there may not be enough room to create an adequately sized opening for the implant without causing distortion or excessive tension on the closure. More importantly, to reach the implant pocket, the incision must pass through the breast gland itself, transecting some milk ducts and nerve pathways. This is why there’s a slightly higher chance of affecting nipple sensation or potentially impacting your ability to breastfeed compared to the inframammary approach. Research also shows a marginally increased risk of capsular contracture (scar tissue hardening around the implant), though this remains relatively uncommon with modern techniques.
Transaxillary Incision (Armpit)
Some patients are drawn to the transaxillary approach because it leaves no scar on the breast itself. Instead, the incision is hidden in a natural crease of your armpit. For patients who are concerned about any mark on the breast, this can be an appealing option.
To insert the implant, I create a tunnel under your skin from the armpit to the breast pocket. This is a highly technical procedure that requires the use of an endoscope (a tiny camera on a thin tube) to guide the dissection and placement. The endoscope allows me to visualize the chest muscle and create the pocket accurately from a distance. The result? No visible boob job scar on your breast tissue at all.
The trade-off is that this approach is technically more demanding and offers less direct control over the final implant positioning compared to the inframammary incision. There’s more tissue manipulation involved, which can occasionally lead to temporary numbness in the armpit area or, rarely, changes in muscle function. It’s also generally better suited to saline implants, which can be filled after insertion, making them easier to maneuver through the tunnel, although silicone implants can be used by highly experienced surgeons.
Because I’m not cutting through breast tissue, this approach typically preserves nipple sensation and doesn’t affect your milk ducts. However, keep in mind that the armpit scar, while hidden when your arms are down, can be visible when you wear sleeveless tops, swimsuits, or raise your arms.
Transumbilical (TUBA) Incision (Belly Button)
The TUBA technique is sometimes marketed as “scarless” breast augmentation because the incision is made inside your belly button, leaving no marks on your breasts at all.
However, this approach comes with significant limitations and is the least common method used by board-certified plastic surgeons today. It only works with saline implants—silicone implants are pre-filled and simply can’t be rolled up and inserted this way. The implants must travel a long distance through a tunnel created from your navel to your breasts. Much of this dissection is done “blind,” without the direct visualization offered by other techniques, which significantly increases the risk of malposition (the implant ending up in the wrong spot, often too high or too close together). The extensive dissection required can also lead to more bruising and swelling.
While your breast tissue and milk ducts remain untouched, the scar inside your navel can sometimes distort its natural shape. Correcting issues that arise from a TUBA procedure can also be particularly challenging. Because of these limitations and higher risks, this is an approach I rarely recommend.
