Case 1 – Bilateral Breast Augmentation with Super Breast Lift

Reason for seeking OC Breast Surgery Specialist:
This 40 year old mother of three from Orange County, California was unhappy with the appearance of her breasts. Her current size was a 32D but she wanted volume in the upper pole and more overall fullness. She wanted, “nice looking breasts, not fake or really big”. Upon examination, the surgeon determined that her breasts were asymmetrical (uneven) with the right breast being lower than the left. He also noted that her right ribcage was slightly more inferior than left which accounted for her right breast being wider in diameter than her left. Additionally, her left inframammary fold (IMF) was slightly higher on the left side than the right. Due to the ptosis (sagging) of her breasts, the surgeon determined she was a good candidate for a lift.

Pre-Op Sizing
The patient was instructed to follow the Breast Implant Sizing Guide at home before her pre-op sizing appointment in our Newport Beach office. This home exercise involves purchasing one or more bras that depict the desired post-op breast size and stuffing them with nylon sacks filled with flax seed. The point of the exercise is to go beyond what is considered ideal volume to something that is too big. This allows the patient to experience what she considers to be too little volume and too much volume.

On the day of the pre-op sizing appointment, the patient brought along her bra and a tight garment like a simple t-shirt for the fitting. With the help of our Clinical Coordinator, the patient tried on an array of gel tapered implants to find her ideal range between 150 cc and 200 cc. Patients are instructed not to focus too much on the number as one’s anatomy will affect the way the implants will project after placement. To further ensure meeting the patient’s expectations, she provided photos of naked breasts in a neutral position from a plastic surgery website like ours that depicted her desired size along with photos depicting breasts she considers too small and too large.

Blood Test
All breast surgery patients are required to have a Mammogram or Ultrasound within 1 year of the surgery date. All patients must have a CBC and Basic Metabolic blood test to be cleared for surgery. Patients over the age of 50 must have had an EKG within 1 year of the surgery date.

Day of Surgery
Bilateral Breast Augmentation with Super Breast Lift is done under General Anesthesia. Surgery time is 4 hours and 30 minutes. Prior to surgery, skin marks were placed with the patient in the sitting and standing position. Implants were placed submuscular (under the muscle) for the most natural result and to reduce the risk of capsular contracture. To accommodate the difference in size of her natural breasts, a 175cc Natrelle Inspira silicone smooth, moderate profile implant was placed on the left side and a 195cc Natrelle Inspira silicone smooth, moderate profile implant placed on the right. Internal and external tissues were tightened and lifted as per the Super Breast Lift technique. The patient’s breasts were wrapped with an Ace Bandage then secured in a post-surgical support bra with surgical tape placed on the sutures to minimize scars and improve healing.

Post Op
The next morning at her first post-op appointment, the ace bandage was removed. The patient was instructed to do the Implant Moving Exercises several times a day for the first two weeks. Post Op photo taken one month after surgery. Scars will continue to improve over the next six to twelve months. The patient loved the size and shape of her breasts after surgery.
