Case 2 – Bilateral Breast Augmentation with Modified Super Breast Lift

Reason for consulting with OC Breast Surgery Specialist
This 41-year-old patient from the Los Angeles area had been referred by a friend who had surgery with an OC Breast Surgery specialist. She wanted to improve the appearance of her 34B cup breast size by increasing the volume to a C cup. The distance from her sternal notch to her nipples was 21 cm on each breast along with more volume in her lower breast curve made her a candidate for a Super Breast Lift. The problem was she was concerned with the scars. Many lift candidates express concern over post-op scars, which do tend to heal well and after six months or more are hard to detect. In this case, the surgeon recommended an implant and a modified Super Breast Lift to avoid the scars around her nipples while still addressing her goals of lifting the breasts and increasing the volume.

Surgical Plan – Bilateral Breast Augmentation with Modified Super Breast Lift

Pre-Op Sizing
The patient was instructed to follow the Breast Implant Sizing Guide at home before her pre-op sizing appointment in our Newport Beach office. This home exercise involves purchasing one or more bras that depict the desired post-op breast size and stuffing them with nylon sacks filled with flax seed. The point of the exercise is to go beyond what is considered ideal volume to something that is too big. .This allows the patient to experience what she considers to be too little volume and too much volume.

On the day of the pre-op sizing appointment, the patient brought along her bra(s) and a tight garment like a simple t-shirt for the fitting. With the help of our Clinical Coordinator, the patient tried on an array of gel tapered implants to find her ideal range which was between 275 cc-350 cc. She felt this would achieve her goals and leave her with the natural sloping breast without too much fullness on top that she desired.

Mammogram
Blood Test
All breast surgery patients are required to have a Mammogram or Ultrasound within 1 year of the surgery date. All patients must have a CBC and Basic Metabolic blood test to be cleared for surgery. Patients over the age of 50 must have had an EKG within 1 year of the surgery date.

Day of Surgery
Bilateral Breast Augmentation with Modified Super Breast Lift is done under General Anesthesia. Surgical time was 3 hrs and 15 minutes. Before surgery, skin marks were placed on the patient in the sitting and standing position. In a Modified Super Breast Lift the surgeon places an incision from the inferior (lower) areolar edge down to the inframammary fold (IMF). There is no incision around the nipples as there would be in a traditional lollipop lift. The lower breast curve dermis and lower breast fold dermis were tightened using the Super Breast Lift techniques. The surgeon placed a 345cc Natrelle Inspira smooth, moderate profile cohesive silicone implant in each breast submuscular (under the muscle). The patient’s breasts were wrapped with an ace bandage then secured in a post-surgical support bra with surgical tape placed on the sutures to minimize scars and improve the healing time.

Post Op:
The next day at her first post-op appointment, ace bandage was removed and the patient was instructed to do the Implant Moving Exercises several times a day for the first few weeks. Post Op photos were taken two months after surgery. The patient was very happy with her results and how her scars were healing. Scars continue to improve for six months to one year after surgery.
