Case 3 – Bilateral Breast Augmentation with Super Breast Lift

Reason for seeking OC Breast Surgery Specialist:
This 45-year-old mother of four traveled from Scottsdale, Arizona to consult with a breast specialist at OC Breast Surgery to improve the appearance of her sagging, deflated 34D breasts. After breast feeding all four of her children, including a set of twins, her breasts had lost volume and perkiness. Upon examination, the doctor determined that she had more volume in her right side than her left. When the doctor simulated a lift, she realized that she wanted more volume in her upper pole. Her current bra size was 34D and she thought she would be happy at a 34DD or 34F after surgery.

Surgical Plan:
Bilateral Breast Augmentation with Super Breast Lift.

Pre-Op: Sizing
The patient was instructed to follow the Breast Implant Sizing Guide at home before her pre-op sizing appointment in our Newport Beach office. This exercise involves filling individual nylon sacks with 100ml or less of flax seed and filling them into a bra until a desirable volume is achieved. This allows the patient to experience too little volume too much volume and what looks just right.

On the day of the pre-op sizing appointment, the patient is instructed to bring a bra and a tight-fitting garment, like a simple t-shirt. Our Clinical Coordinator assists the patient in placing external sizers into the bra to find the proper range of implant size. At home she was happy with 235ml of flax seed, but in the office she chose a 350cc implant as her favorite. The Breast Implant Sizing Guide is not an exact science but rather an opportunity for the patient to begin visualizing herself with larger breasts. The final step in sizing is for the patient to provide photos of breasts that are just right, too big, and too small. Patients are encouraged not to get fixated on the number as one’s anatomy will affect the way implant project after placement.

Pre-Op Labs
All breast surgery patients are encouraged to have a Mammogram or Ultrasound within 1 year of the surgery date. All patients must have a CBC and Basic Metabolic blood test to be cleared for surgery. Patients over the age of 50 must have had an EKG within 1 year of the surgery date.

Surgery:
Bilateral Breast Augmentations with Super Breast Lifts are done under general anesthesia. Surgical time was 4 hours 30minutes. Skin marks were placed on the patient in the sitting position before surgery as shown in the pre-op photos. After the surgeon created the capsule for placing the implant below the muscle, he tried various internal sizers, each time sitting the patient up on the OR table to see how the breasts looked when they fell naturally. To achieve her desired result, the surgeon placed a 330cc Natrelle silicone smooth, moderate profile, Inspira cohesive implant in the larger right-side breast and a 360cc Natrelle silicone smooth, moderate profile, Inspira cohesive on the left side. Internal and external tissues were tightened and lifted as per the Super Breast Lift technique. The breasts were secured in a post-surgical bra and surgical tape placed on the sutures to minimize scars and improve healing time.

Post Op:
The next day at her first post-op appointment, the patient was instructed to follow the Breast Implant Moving Exercises several times a day for the first two weeks. Post-op photos taken five months after surgery. Scars continue to improve for one year after surgery. The patient is extremely happy with her perkier, more youthful looking breasts.
