Case 5 – Bilateral Breast Augmentation with Super Breast Lift

Reason for consulting with OC Breast Surgery specialist
This 45 year-old Huntington Beach patient was unhappy with her sagging breasts after three pregnancies followed by breastfeeding. Her current bra size was a 34D or a 36C, but she lacked projection, and all her volume was in the lower pole. She wanted more breast fullness on top but still wanted to look as natural as possible. In her exam, the surgeon determined her left breast was larger in volume than her right. The distances between her sternal notch (collar bone) and nipple areolar complex was 24.5 cm on the right side and 23.5 cm on the left side.

Surgical Plan – Bilateral Breast Augmentation with Super Breast Lift

Pre-Op Sizing
The patient was instructed to follow the Breast Implant Sizing Guide at home before her pre-op sizing appointment in our Newport Beach office. This home exercise involves buying one or more bras of the desired post-op size and stuffing them with nylon sacks filled with flax seed or something equivalent. The point of the exercise is to find the volume that feels the most comfortable then continue to stuff the bra until reaching a volume that feels too big. This helps the patient to experience what she considers to be too little volume, too much volume, and just the right amount of volume. This patient preferred 225 cc for her R breast and 175 cc for the L breast.

On the day of her pre-op sizing appointment, the patient brought her bras and a tight garment to try on with the gel tapered external sizers. Sometimes patients bring a loved one along to the appointment to offer input, and our Clinical Coordinator is there to assist if needed, but it is really the patient’s moment to decide what she thinks looks best on her body. This patient decided on a range of 200 cc -250 cc for her right breast and 150cc -200 cc for her left breast. Patients are instructed not to focus too much on the number as there are about 22 different implant sizes from 150 cc to 800 cc and the majority of those only differ in volume by as little as two tablespoons. This is why it is important for patients to submit photos of naked breasts in a natural position from plastic surgery sites like ours to depict the ideal breast size along with breasts they feel are too big and too small.

Pre-Op Labs
Blood Test
All breast surgery patients are required to have had a Mammogram within 1 year of the surgery date or an Ultrasound. All patients must have a CBC and Basic Metabolic blood test to be cleared for surgery. Patients over the age of 50 must have had an EKG within 1 year of the surgery date.

Day of Surgery
Breast Augmentation with Super Breast Lift is done under General Anesthesia. Surgery time is 4hrs and 30min. Skin marks were placed with the patient before surgery in the sitting position prior to surgery. After the patient was put to sleep and the breasts prepped and draped in sterile fashion, the surgeon created a generous submuscular pocket on each side for the implant placement. Various sizers were tried without the patient in the sitting position multiple times before it was decided that 275 cc on the right and 240 cc on the left gave her the volume she desired. This was done with the proposed Super Breast Lift incisions approximated with staples and later once the incisions had been sewn. Once the sizers had been decided and the incision, the patient was sat upright on the OR table to check that the breasts had nice contours and symmetry. The staples were removed, and the triple dermal tightening technique of the Super Breast Lift applied. Once again the patient was checked in the sitting position for shape and volume. The sizers were removed and the Natrelle Inspira silicone cohesive smooth moderate profile implants placed under the muscle. The skin was closes with sutures and skin tape placed over the incisions. The chest was wrapped in an ACE bandage.

Post Op
The next day at her first post-op appointment the patient was instructed to follow the Breast Implant Moving Exercises several times a day for the first two weeks. At her six week post op the implants had relaxed into place and were moving well. Post-op photos were taken six weeks after surgery. Scars will continue to improve for one year after surgery.
