Case 2 – Bilateral Breast Augmentation with Left Crescent Lift

Reason for consultation with OC Breast Surgery Specialist
This 29 year old patient from Los Angeles, California came to the OC Breast Surgery center to improve the appearance of her small breasts. She wanted to go from a small A/B cup to a full C or small D cup. She also indicated that she wants to look “fuller on top as opposed to concave.” She indicated that she likes a natural looking breast but did not want them to hang too low.
Upon exam, the doctor determined that the volume of her current breast tissue appeared to be symmetrical but that her left nipple areolar (NA) complex was slightly lower than the right.

Surgical Plan – Bilateral Breast Augmentation with Left Crescent Lift

Pre-Op: Sizing
The patient was instructed to follow the Breast Implant Sizing Guide at home before her pre-op sizing appointment in our Newport Beach office. This home exercise involves purchasing one or several bras of your desired post surgery size and stuffing them with nylon, flax seed filled sacks. Patients are encouraged to fill the bra until reaching a desired volume then go bigger until reaching a volume considered too big.

On the day of the pre-op sizing appointment in our Newport Beach office, the patient brought along her bras and a simple t-shirt. With the help of our Clinical Coordinator, she placed several tapered gel sizers into her bra to find the proper range of implants. This patient decided on a range between 400 cc and 450cc. She also provided several photos of naked breasts from our website that depicted her desired size along with photos of breasts that were too small and too big. Patients are instructed not to focus too much on the number as one’s anatomy will affect the way the implants will project after placement. The Clinical Coordinator noted that the patient preferred a “little bit of a fake shape” with regard to the fullness on top.

Blood Test
All breast surgery patients are required to have had a Mammogram or Ultrasound within 1 year of the surgery date. All patients must have a CBC and Basic Metabolic blood test to be cleared for surgery. Patients over the age of 50 must have had an EKG within 1 year of the surgery date.

Day of Surgery
Breast Augmentations with Left Crescent Lift are done under General Anesthesia. Surgical time is 2 hrs 15 min. Before surgery, skin marks were placed on the patient in the sitting position. A 4 cm incision was made along the inframammary fold of each breast. Dissection was carried down to the muscle. Tissue was removed off the inferior aspect of the muscle, and the muscle divided. A generous submuscular pocket was created for implant placement (under the muscle). Various internal sizers were tried and the patient sat upright on the OR table to visualize the natural contours of the breast. After placing 405 cc sizers, the surgeon sat the patient upright and determined that her tissues did not appear to be able to go any fuller and looked to be a good size similar to her preoperative photo desires. Before placing the 405 cc Natrelle smooth moderate profile Inspira cohesive implants through a sterile Keller Funnel, the pockets were irrigated with a Triple Antibiotic solution. The surgeon made a small “crescent moon shaped” incision over her top left nipple to slightly raise the nipple line for symmetry. The patient’s breasts were wrapped with an ace bandage then secured in a post-surgical support bra with surgical tape placed on the sutures to minimize scars and improve the healing time.

Post Op
The next morning at her first post-op appointment, the ace bandage was removed and she was instructed to follow the Breast Implant Moving Exercises several times a day for the first two weeks. This patient wore a strap over her breasts to encourage them to move down and into position. Post-op photos were taken 2 months after surgery. Patient is extremely happy with the size and shape of her breasts.
