Case 4 – Bilateral Breast Augmentation

Reason for consulting with OC Breast Surgery specialist
This 27-year-old from the LA area of Southern California wanted to increase the volume of her breasts and have more cleavage. The surgeon consulted with her about breast augmentation surgery, discussing the two important characteristics of breasts that can be altered in surgery: shape and volume. The shape of the breast is determined by the external skin envelope and the volume is whatever is inside the skin envelope (fat, breast tissue, and possibly an implant). To increase cleavage, the surgeon explained that in general a wider diameter implant (moderate profile) helps to fill out the cleavage and upper pole better than a narrower diameter or high profile implant. Lastly, they discussed the type of incision and she chose the inframammary fold (IMF) because that offered her the best option should she decide to one day have children and breastfeed.

Surgical Plan – Bilateral Breast Augmentation

Pre-Op Sizing
The patient was instructed to follow the Breast Implant Sizing Guide at home before her pre-op sizing appointment in our Newport Beach office. This home exercise involves purchasing one or several bras of your desired post surgery size and stuffing them with nylon, flax seed filled sacks. Patients are encouraged to fill the bra until reaching a desired volume then go bigger until reaching a volume considered too big. She was happiest with a volume of 600 cc with her at home sizing exercises.

On the day of the pre-op sizing appointment in our Newport Beach office, the patient brought along her bras and a garment for fitting. With the help of our Clinical Coordinator, she placed several tapered gel sizers into her bra to find the proper range of implants. The range she chose was 550 cc to 600 cc. This also matched the photos she chose of naked breasts from ours and other plastic surgery sites to depict her ideal size along with breasts that were too small and too large.

Pre-Op Labs
Blood Test
All breast surgery patients are required to have had a Mammogram or Ultrasound within 1 year of the surgery date. All patients must have a CBC and Basic Metabolic blood test to be cleared for surgery. Patients over the age of 50 must have had an EKG within 1 year of the surgery date.

Day of Surgery
Breast Augmentation surgery is done under General Anesthesia. Surgical time is 2 hrs. Skin marks were placed with the patient in the sitting position prior to surgery. In surgery, incisions were placed on each side in a symmetric fashion along the inframammary fold. Dissection carried down the muscle. Tissue removed from the inferior aspect of the muscle and the muscle divided. Generous submuscular pocket was created. Same thing was carried on the opposite side without deviation, and then, 550 cc sizer was placed. The surgeon noted that 550 cc was the largest implant able to fit into the skin envelope. The surgeon enhanced the border around the breast by removing 75 mL of fat from the anterior axilla and lateral chest wall via liposuction.

Post Op
The next morning at her first post-op appointment, the patient was instructed to follow the Breast Implant Moving exercises several times per day for the first few weeks until the implants settled into place. At two months when the post-op photos were taken, the patient was very happy with the results.
