Patient 05 - Leaking Saline Implant

Case 5 – Bilateral Breast Revision with Exchange of Implants and Super Breast Lift

Reason for consulting with OC Breast Surgery specialist
This 59-year-old had a left, leaking saline implant from a previous surgery more than twenty years ago. Saline ruptures present very quickly and noticeably. This patient reported that one day she noticed her left side completely flat. A saline rupture is not a medical emergency even though it can be disconcerting. Saline is non-toxic to the body and is simply absorbed by the body over time. Before the rupture, the patient was happy with the way her breasts looked and said, “If I didn’t have the rupture, I would not be touching my breasts because I still love how they look”. The surgeon gave her the option of replacing only the ruptured left implant, but after further discussion she decided that she would like smaller, more perky breasts.

Surgical Plan – Bilateral Breast Revision with Exchange of Implants and Super Breast Lift.

Pre-Op Sizing
Since this patient was choosing to have a smaller implant, she did not do any pre-op sizing. Instead, she provided photos of her ideal breasts, breasts that were too big, and those that were too small.

All breast surgery patients are required to have a Mammogram or Ultrasound within 1 year of the surgery date. All patients must have a CBC and Basic Metabolic blood test to be cleared for surgery. Patients over the age of 50 must have had an EKG within 1 year of the surgery date.

Day of Surgery – Bilateral Breast Revision with a Super Breast Lift procedures is done under General Anesthesia. Surgery time is 4 hrs and 30 min. Before surgery, skin marks were placed with the patient in the sitting position. After removing her implants and tightening the capsule, various internal sizers were placed through a sterile Keller Funnel and the patient sat up in surgery to see how the breasts fell naturally. Natrelle Inspira smooth, moderate profile cohesive silicone implants were placed (310cc-R, and 330cc-L) under the muscle.. Internal and external tissues were tightened and lifted as per the Super Breast Lift technique. The patient was placed in a post-surgical bra and surgical tape placed on the sutures to minimize scars and improve healing.

Post Op
The next day at her first post-op appointment, the patient was instructed to follow the Breast Implant Moving Exercises several times a day for the first two weeks. Post-op photos taken one month after surgery. The rippling noticed in the photo is from swelling that made her stretch marks more prevalent. Once the swelling subsided, the breast appeared smooth. Scars will continue to improve for one year after surgery. The patient loves her new, smaller, perkier breasts.
