Case 1 – Bilateral Breast Revision with Implant Exchange and Super Breast Lift

Reason for consulting with OC Breast Specialist
This 61 year old patient from Southern California wanted a breast revision to reduce the size of her 34/36 DD breasts. After losing weight, she desired to be lifted and more balanced with her body. Her previous surgery was in 1987 by a surgeon not associated with OC Breast Surgery. She did not know her current implant size. She also did not know that both of her implants were ruptured. Her mammogram was normal, and she was not experiencing any noticeable changes in her breasts.

Surgical Plan– Bilateral Breast Revision with Implant Exchange and Super Breast Lift

Pre-Op Sizing
Since the patient desired smaller implants, sizing before surgery was not necessary. She provided photos of her desired breast size along with photos of breasts that were too big and too small.

Pre-Op Labs
Blood test
All breast surgery patients are required to have a Mammogram or Ultrasound within 1 year of the surgery date. All patients must have a CBC and Basic Metabolic blood test to be cleared for surgery. Patients over the age of 50 must have had an EKG within 1 year of the surgery date.

Day of Surgery
Breast Revision with Exchange of Implants and Super Breast Lift are performed under General Anesthesia. Surgery time is 4 hrs and 30 min.

Prior to surgery the patient was marked in the sitting position. In surgery, the surgeon entered the capsule to remove the previous implants and the ruptures were discovered. The silicone was removed, and the capsule wiped clean. (See photos of ruptured implants). The surgeon discovered a posterior tag that read, “McGhan 400cc” in each breast indicating the brand and size of her previous implants. Internal sizers were placed through sterile Keller Funnels and the patient sat upright in surgery to see how the breasts fell naturally. The surgeon placed Natrelle Inspira silicone smooth moderate profile implants (445 cc in her right breast and 375 cc in her left breast) then performed the Super Breast Lift triple dermal technique that gave the patient the more youthful breasts she desired. To further improve the look of the breast, the surgeon liposuctioned her lateral breast and chest wall removing 150 cc of fat from her right side and 200 cc of fat from her left side . She was placed in a post-surgical bra and her incision covered in skin tape to minimize the risk of scars.

Post Op
The next day at her first post-op appointment, the patient was shown the implant moving exercises and instructed to apply those every few hours to minimize the risk of capsular contracture. Her photos are taken just two weeks after surgery to show how well she is healing without any need for drains. The tape stays in place for the first six to eight weeks depending on the individual. There is no need to apply scar creams.
