Case 3: Breast Revision with Exchange of Saline Implants and Super Breast Lift

Reason for Seeking OC Breast Surgery Specialist
This 52- year-old South Orange County woman wanted smaller breasts that were lifted and more youthful looking. She also felt like her right implant had been placed upside down.

Her original breast augmentation was in 1995 when she got saline implants. In 2018 she had her implants replaced with new saline implants because she thought they were getting old. She was having no problems with her implants at that time. While many doctors suggest replacing implants every 10 years, OC Breast Surgery specialists do not recommend replacing implants unless the patient is having a problem with one or both like capsular contracture or a rupture or the patient is unhappy with how her breasts appear.

When she had her implants replaced in 2018, she wanted to go smaller then but was told she would have to have a lift and at that time was not ready. After speaking with an OC Breast Surgery specialist and learning about the no-drain Super Breast Lift, she felt confident and ready to move forward with a lift.

Pre-Op Sizing
Since the patient desired smaller implants, no pre-op sizing was necessary. She provided photos of her desired breast size along with photos of breasts that were too small and too big.

Pre-Op Labs
Blood test
All breast surgery patients are required to have a Mammogram or Ultrasound within 1 year of the surgery date. All patients must have a CBC and Basic Metabolic blood test to be cleared for surgery. Patients over the age of 50 must have had an EKG within 1 year of the surgery date.

Surgery – Breast Revision with Exchange of Implants and Super Breast Lift are performed under General Anesthesia in our outpatient surgery center. Surgical time is 4 hrs 30 min.

Prior to surgery skin marks were placed with the patient in the sitting position. When removing the Silimed smooth 510MP saline implant the surgeon discovered that the right implant had in fact been placed upside down. Because this type of implant has a very flat surface it would make it quite noticeable in the lying down position. Various sizers were tried and the patient sat upright in surgery to see how the breasts fall naturally to determine desired size. It was decided to use 420cc saline implants that were placed on top of the muscle. The surgeon then performed the Super Breast Lift with its triple dermal technique to lift the breasts and make them more youthful. The patient was placed in a surgical bra and skin tape placed on the incisions to minimize scars.

The next day at her first post-op appointment, the patient was instructed to follow the Breast Implant Moving exercises every few hours for the first several weeks to minimize the risk of capsular contracture and help the implant settle into place. Post-op photos were taken 4.5 months after surgery. Scars continue to improve up to one-year post-op.
