The patient decided on silicone as it offers the most natural feel and look. She was comfortable with the scars of a lift and excited to learn that the Super Breast Lift required no drains post-operatively.

Initially, she and the surgeon discussed a breast revision with implant exchange. They talked about changing to silicone implants, which can be a concern for some patients. Silicone implants today are much improved from previous generations which were more prone to bleeding (silicone leaking on the outside of an intact shell) or rupture. Silicone implants today are made of a very cohesive gel and if the shell fails, the gel will generally stay put inside the implant.

The shape of the breast is determined by the external skin envelope and the volume is whatever is inside the skin envelope (fat, breast tissue, and possibly an implant). To increase cleavage, the surgeon explained that in general a wider diameter implant (moderate profile) helps to fill out the cleavage and upper pole better than a narrower diameter or high profile implant. Lastly, they discussed the type of incision and she chose the inframammary fold (IMF) because that offered her the best option should she decide to one day have children and breastfeed.

She was flat chested but had very large breasts that hung down to her waistline. She reported her current breast size as a 36H and wanted to be a 36D/DD. Upon examination, the surgeon measured the distance from her sternal notch (collar bone) to her nipple areolar complex as 32cm. A general guideline is any measurement over 20 cm, plus or minus a few centimeters, qualifies as a breast lift candidate. In this case she was a full 12cm beyond what is considered a breast lift candidate. The greater the distance, the more the nipple areolar complex has to be lifted which increases the risk of blood supply to the nipples. Her surgeon discussed these risks and the possibility of needing a nipple areolar graft or possibly a Star Flap Nipple Reconstruction. The surgeon also discussed adding a fat transfer to the upper pole to correct its flat appearance.. A small implant could account for more upper pole fullness, but the patient did not want implants. The patient was made aware that fat transfers are unpredictable in how long and how well they last.

Reason for consulting with OC Breast Surgery Specialist
This 51-year old mother of two from Orange County, California has always wanted to have small, cute and perky breasts so that she could wear a dress without wearing a bra. While she had gained a few pounds over the years, she reported that her breasts have always been too big. In 2000 she had a breast augmentation with saline implants to help fill up the skin envelope. In 2007 she had a breast revision and exchanged her implants for silicone implants. She also had a circumareolar lift. Both of her previous surgeries were with surgeons not associated with OC Breast Surgery. During her consultation, the surgeon discussed reducing the tissues as much as possible and removing her implants, but she wanted fullness in her upper pole so she decided on the smallest implant possible for shape.

No pre-op sizing was necessary. The patient submitted photos of her desired size and photos of breasts that were too big and too large. She reported her current breast size of 38DDD and wanted to be a 38 small C after surgery.

Breast Reduction with Implant Exchange is done under General Anesthesia. Surgical time was 5 hrs. Prior to surgery, skin marks were placed with the patient in the sitting position. During surgery, the surgeon removed an intact Mentor smooth gel 300cc implants from each breast. He then performed a superomedial capsulotomy and further tightened the capsule with ball cautery. Internal sizers were placed through sterile Keller Funnels and the patient sat upright on the operating table so the breasts could fall naturally. A Natrelle Cohesive Inspira 195cc silicone, smooth moderate profile implant was placed in each breast subglandular, under the muscle. To reduce the size of her breasts beyond exchanging her larger implants to smaller ones, the surgeon removed tissues surgically and with liposuction for a total of 605g from the right breast and 505g from the left. To lift the breasts for the “perky” look she wanted, the surgeon performed the internal triple dermal tightening techniques of the no-drain Super Breast Lift. The patient was placed in a surgical support bra and skin tape placed on her incisions.
Post Op
The next morning at her first post-op appointment, the patient was instructed to perform the breast implant moving exercises every couple of hours for the first two weeks as the implants settled into position. He explained that there is a trade out for lifted breasts but that the tissues in the breast tend to heal very well, though everyone is different.

Crescent lift can be a good option for some patients, but it does not lift the underlying tissues of the breast. It only makes the breasts appear to be lifted by raising the nipple up a little.
